Our team will evaluate your nutritional and healthcare needs. We will determine if your Body Mass Index (BMI) will qualify you for Medical management of your weight. NuCare Medical Providers will use nutritional guidelines to assist you in making healthier decisions on your diet selections.

Virtual visit that allows you to see and hear the provider or an in office visit. If you have vital sign capabilities they will be needed during the virtual visit( i.e. blood pressure, temperature, height and weight. Discuss your chronic or acute health needs. TB skin test (read only)

Schedule a visit to have your TB skin test read when you are unable to return to your PCP. This is uncommon, but if needed we are here for you. contact us. We can provide you with the necessary document. Weekend On-Demand hours available as well. Contact us.
Weight loss consultation

Have questions concerning your qualification for our weight loss program but are still on the fence about it? Schedule a consultation to have your questions answered. Virtual appointment 15 minutes. No prescription.
Weight loss visit basic(in office only)
$100.00
This includes your visit with our Nurse Practitioner and a FDA approved medication. Options are Phentermine, Orlistate, Qysmia, Contrave, and Plenity prescription. Per Guidelines- You must have a BMI of 30 or higher (For an otherwise healthy person with a BMI 27 or higher for a person with at least 1 co-morbidity). Prescription (per provider assessment of choice of FDA approved medication). Plenity approved BMI is 25-40.
Weight loss-gold package (in office)

Interested in other weight loss medications? If you qualify, a personalized weight loss package will be devised addressing nutritional and medical management with GLP-1 options such as Saxenda and Wegovy sample or prescription. (See BMI qualifications above). If you have medical insurance, please call to verify coverage of medication. Call us for more information on patient assistance options, ect.
Nexus letter visit

Military Service members are invited to see us for their Nexus letter needs. We will review your records and provide you with the documents you need from our providers. One half payment must be submitted at the time of the initial consultation.
Nexus letter visit + Exam

Military Service members are invited to see us for their Nexus letter needs. Transitioning to civilian life is not easy. During this visit we will complete a thorough review of past and current health issues. We will review your records and provide you with the documents you need from our providers. We will also do an exam for any issues that may be service related and refer you to specialist to confirm the diagnosis.
All fees are non-refundable and are collected before and at the time of visit.
